Fill out the NEW PATIENT PACKET found below and return it to our office. We encourage you to include your license and insurance information as well. We will need records from your most recent PCP, any urgent care or ER visits, and any records from specialists you have seen over the past few years. If you need additional release forms for multiple provider locations, that can be found at the bottom of this page.
We will send your records request to your previous providers and once we receive those records, we will have the provider you are requesting review them. Please note this process can take several weeks depending on how long it takes to get your records. (Please note, requesting to become a new patient at more than one Family Practice at a time will not effect your request to HFP.)
Once our providers have an opportunity to review your records, they will let us know if they are able to accept you as their patient. Depending on the current established patient load/patient acuity, the provider you requested may not be able to accept you at this time. We strive to provide the best care and ensure we are able to see our established patients in a timely manner, so this sometimes means limiting new, incoming patients. We understand this can be frustrating for those that are not accepted.
Once a response has been received, we will contact you at the number on your request forms. Again, we strive to make this process as quick as possible, but it can take longer due to unexpected delays.
If you are accepted by a provider in our office, we encourage you to make an appointment at your earliest convenience. Our nurse practitioners are not able to see new patients for their first visit. Scheduling your first visit with one of our doctors will ensure you are able to schedule an appointment with MacKenzie Cheek, NP-C.
There will be NO changes to the Saturday morning walk-in clinic.
